We are conveniently located in the IBC Medical Plaza in Irvine, CA near Diamond Jamboree, The District and John Wayne Airport. Selected Saturdays appointments available for LASIK consultation and postoperative visits.

Prior to your first visit with us, please print, complete the CVI Patient Registration and CVI Health History, and bring these forms with you.

If you would like us to bill your medical insurance (e.g. office visits, cataract surgery, etc.) and/ or vision insurance (e.g. for eye exams, glasses, or contact lenses), please let us know when making your appointment.

Patient payment including copay and deductables are due at time of services rendered. We accept credit cards; and offer Care Credit and Chase Healthcare Financing on all surgical procedures including Custom IOLs, LASIK and PRK vision correction.
